Step 1

Initial assessment conducted to identify needs, challenges, capabilities and assign appropriate targets/objectives for development.

Step 2

Understand Health/Medication needs and make provisions accordingly

Step 3

Build an appropriate independent development/progression plan with SMART objectives (Specific, Measurable, Achievable, Realistic & Time Bound)

Step 4

Build a schedule of activities (aligned to Pt.2) inclusive of onsite/offsite activities appropriate to the needs, desire and capabilities of the individual

Step 5

Set appropriate time schedule for periodic review and track progression (i.e. 3monthly, 6monthly, 12montly)
